The form below is used to nominate an individual for emergency financial assistance through the Generosity Fund. This form must be completed and submitted by a General Leader.
To be considered for funding, nominees must meet the following criteria:
Required Documentation:
All supporting documentation must be uploaded at the time of submission. This may include rental agreements, late notices, invoices, receipts, legal notices, or other relevant documents. The nominee’s name must be clearly visible on all documents submitted (not just totals or summaries). *Incomplete submissions may delay review.
All information submitted through this form will be treated in accordance with the BMD Confidentiality Agreement.
