If additional support is needed, nominations for the Generosity Fund are submitted by a General Leader.

Instructions
The form below is used to nominate an individual for emergency financial assistance through the Generosity Fund. This form must be completed and submitted by a General Leader.
To be considered for funding, nominees must meet the following criteria:
- Current employee of a Burrell Family McDonald's
- Has not received funding from the Generosity Fund this calendar year
- Demonstrated need for emergency relief to assist with the following expenses for the employee or one of their dependents:
- Medical bills for emergency treatment or surgeries
- Mental and behavioral health services
- Avoiding eviction or foreclosure
- Overdue utility bills
- Providing food and other necessities for young children
- Fees associated with adoption
- Other similar situations of documented need, as appropriate
Required Documentation:
All supporting documentation must be uploaded at the time of submission. This may include rental agreements, late notices, invoices, receipts, legal notices, or other relevant documents. The nominee’s name must be clearly visible on all documents submitted (not just totals or summaries). *Incomplete submissions may delay review.
All information submitted through this form will be treated in accordance with the BMD Confidentiality Agreement.
